What Botox actually does, and what it doesn't

BOTOX® (onabotulinumtoxinA) is an FDA-approved neurotoxin injectable that temporarily relaxes the facial muscles responsible for expression-related wrinkles.

When you squint, frown, or raise your eyebrows, specific muscles contract and create lines in the overlying skin. Over years of repetition, these lines become permanent, visible even at rest. BOTOX® intercepts the signal between the nerve and the muscle, allowing the skin to smooth out and preventing the further deepening of those lines.

BOTOX® does not freeze your face, erase all expression, or treat wrinkles caused by volume loss or gravity, those require different tools. One of the most FDA-approved treatments in modern medicine. Since its first approval decades ago, BOTOX® has been studied extensively and its approved uses have steadily grown. BOTOX® Cosmetic is now the only product FDA-approved for four aesthetic indications: frown lines, crow's feet, forehead lines, and, as of 2024, platysma (neck) bands. The same molecule, as BOTOX®, is also FDA-approved for a range of medical conditions including chronic migraine, cervical dystonia, and severe underarm sweating. That breadth of approval, backed by decades of clinical study, is a big part of why BOTOX® is so widely trusted.

Botox for neck bands & jawline (platysma)

Everyday movement of the platysma, a broad sheet of muscle across the neck, can create prominent vertical bands and pull down on the jawline, blurring its definition. In October 2024, BOTOX® Cosmetic became the first and only neurotoxin FDA-approved to treat moderate to severe platysma bands, relaxing those cords to soften the neck and help restore a cleaner jawline. The approval was based on two large, placebo-controlled clinical trials, and dosing is customized to the severity of your bands.

What it does and does not do: treating platysma bands softens the vertical neck cords and can sharpen the jaw and neck contour, but it does not tighten loose skin or replace a surgical neck lift. At your consultation we will assess whether a neuromodulator alone is right for you, or whether it is better paired with another treatment.

Consultation & mapping (5–10 min)
Your provider evaluates your muscle movement, skin quality, and goals. We map injection points and discuss exact unit placement. You'll see our plan before any needle is picked up.

Treatment (10–20 min)
Neurotoxin is injected using ultra-fine needles at precise muscle points. Most patients describe a mild pinching sensation. No numbing cream is typically needed, though it's available on request.

Post-care instructions (5 min)

Minimally. Botox is administered with very fine gauge needles and most patients describe a brief pinching or stinging sensation at each injection site. The forehead and crow's feet are typically the least sensitive areas; the glabellar region (11s) is slightly more so. Treatment is complete in 10 to 20 minutes. Numbing cream or cold compresses are available on request for patients who are particularly sensitive to needles.
Initial results typically appear within 3 to 5 days as the muscle begins to relax. The full effect is visible at 10 to 14 days, which is why we schedule your complimentary follow-up at the two week mark. Duration averages 3 to 4 months, though many long term patients find results extend to 4 to 5 months over time as the muscle gradually weakens with consistent treatment. Factors that affect longevity include your individual metabolism, activity level, and the specific area treated.
The number of units depends on your anatomy, muscle strength, and goals. Common reference ranges include: Forehead: 10 to 20 units · Glabellar (11s): 20 to 30 units · Crow's feet: 10 to 15 units per side. A full upper face treatment typically uses 40 to 60 units. First time patients often receive conservative dosing to ensure natural movement. Your provider will explain exactly what they are recommending and why. We believe in transparency, so you will always know your exact unit count rather than being quoted a flat price per area.
Botox has been FDA approved for cosmetic use since 2002 and has one of the most extensive safety records in aesthetics. Common and temporary side effects include mild redness, swelling, or bruising at the injection sites. Rare side effects include temporary asymmetry, headache, or in cases of poor placement, temporary eyelid drooping (ptosis). This is why provider skill and anatomical knowledge are vital. Botox relaxes muscles to address "dynamic" wrinkles caused by movement, such as forehead lines and crow's feet. Dermal fillers add volume to areas that have lost tissue due to aging, such as the cheeks, lips, and under eye hollows. The two are often used together for a "liquid facelift" effect because they address different aspects of facial aging. Your provider will recommend the combination that is appropriate for your anatomy.
Your face simply returns to its pre-treatment baseline as the effect wears off and muscle movement resumes. Botox does not cause additional wrinkles or stretch your skin. Many patients find that after years of consistent treatment, their wrinkles have actually improved even if they stop, because the skin was protected from repetitive folding for a long period.
