Why submental fat is so stubborn, and how to eliminate it

A double chin (submental fat) forms when a layer of fat accumulates below the chin. It can affect people of any weight and is often resistant to diet and exercise.

Submental fat is a genetically influenced fat compartment. Some individuals accumulate fat here regardless of their overall body weight, and others find it is one of the last areas to respond to weight loss. The submental area is also affected by skin laxity, as the overlying skin loses elasticity with age, even a small amount of fat can appear more prominent.

At Florida Aesthetics & Wellness, we offer two clinically proven non-surgical options to eliminate submental fat: Kybella, an injectable deoxycholic acid that destroys fat cells, and CoolSculpting, which uses cryolipolysis to freeze and naturally eliminate fat. Both produce permanent fat reduction, once cells are destroyed, they do not return.

Kybella is an FDA-approved injectable containing synthetic deoxycholic acid, a naturally occurring molecule that destroys fat cell membranes. Injected directly into the submental fat, Kybella permanently eliminates fat cells in the treated area. Results develop over 6–8 weeks as the body processes the destroyed cells. Most patients require 2–4 sessions spaced 6–8 weeks apart.

CoolSculpting

Best for: non-injectable fat freezing under the chin

CoolSculpting uses controlled cooling (cryolipolysis) to freeze and destroy submental fat cells without injections. The CoolMini applicator is designed specifically for the chin area. Both Kybella and CoolSculpting permanently reduce submental fat, but they work differently. Kybella is an injectable, deoxycholic acid is injected directly into the fat, destroying fat cell membranes. It involves more post-treatment swelling (5–10 days) and requires multiple sessions. CoolSculpting freezes fat cells using a non-invasive applicator applied to the skin surface. It is painless during treatment and involves no injections. Your provider will recommend the best option based on the amount of fat, your anatomy, and your personal preferences.

For Kybella, most patients require 2–4 sessions spaced 6–8 weeks apart, depending on the volume of submental fat. Some patients with mild accumulation see excellent results in 2 sessions; those with more significant volume may need 3–4. For CoolSculpting, the submental area typically requires 1–2 sessions with the CoolMini applicator. Your provider will assess your specific anatomy and estimate the number of sessions at your complimentary consultation.

Yes. Both Kybella and CoolSculpting destroy fat cells permanently, the body eliminates them and they do not regenerate. However, it is important to maintain a stable weight after treatment. Significant weight gain can cause remaining fat cells in the area (and elsewhere) to expand, diminishing the visible result. Maintaining a healthy lifestyle preserves your results long-term.

Kybella causes significant swelling in the submental area that typically peaks at 48–72 hours and resolves over 5–10 days. Many patients plan treatments around social commitments for this reason. CoolSculpting has minimal downtime, numbness, mild redness, and firmness in the treated area that resolves within 1–3 days. There is no injection-site swelling with CoolSculpting.

For most patients with good underlying skin elasticity, fat reduction alone produces excellent results with the skin naturally contracting as the fat diminishes. Patients with significant skin laxity from aging or prior weight loss may benefit from a complementary skin tightening treatment. Your provider will assess skin quality at your consultation and recommend whether a combination approach is appropriate.
