About This Treatment

What is HRT pellet therapy?

Pellet therapy delivers bioidentical hormones through small, rice-sized pellets inserted just under the skin, providing steady, consistent hormone levels for 3–6 months without daily medication.

Unlike injections (which create peaks and troughs) or daily creams and pills (which require compliance), pellets release hormones gradually and steadily, keeping levels consistent between insertions. This results in more consistent symptom relief and avoids the mood and energy fluctuations some patients experience with other delivery methods.

About compounded hormone pellets

The hormone pellets we use are compounded. The individual hormones are FDA-approved ingredients and the compounding pharmacy is FDA-registered and inspected, but the finished pellet itself is not an FDA-approved product. That is a real distinction, and we would rather you hear it from us. The same is true of most hormone pellets offered anywhere. Your provider will walk you through what it means for you, and FDA-approved delivery options are available if you prefer one. See our guide to bioidentical versus synthetic hormones for more.

The pellet insertion procedure is simple, performed in-office under local anesthesia in about 15 minutes. Most patients feel no discomfort after the procedure. Pellets are re-inserted every 3–6 months for women and every 4–6 months for men.

What to Expect

What happens during a pellet insertion?

1
Pre-insertion labs (1–2 weeks before)
Hormone levels are checked to ensure accurate dosing. Your provider calculates your personalized pellet dose.
2
Numbing and preparation (5 min)
A small area (typically the upper buttock/hip area) is cleansed and numbed with local anesthetic. You'll feel a pinch from the numbing injection, that's usually the most uncomfortable part.
3
Pellet insertion (5 min)
A small trocar creates a tiny channel through the skin. The pellet(s) are inserted and the site is closed with a small adhesive bandage. No stitches required.
4
Recovery (24–48 hours)
Avoid strenuous lower-body exercise and swimming for 24–48 hours. Some bruising or tenderness at the insertion site is normal. Most patients resume all activities within 2 days.
Common Questions

HRT Pellet Therapy, Frequently Asked Questions

The local anesthetic makes the insertion virtually painless. Most patients say the numbing injection itself is the most uncomfortable part, a brief pinch. The insertion procedure itself is generally not felt.
Most patients notice improved energy and mood within 2–4 weeks as the pellet dissolves and hormone levels optimize. Full effect is typically reached at 4–6 weeks.
Pellets cannot be easily removed once inserted, this is one consideration to discuss with your provider. If you have concerns about a particular hormone, your provider may recommend starting with a lower dose or an alternative delivery method.
Women typically re-insert every 3–5 months; men every 4–6 months. Your provider will monitor your labs and symptoms to determine the optimal re-insertion timing for you.
