Understanding the Concern

Melasma requires a careful, layered approach: not a one-time fix

Melasma is one of the most challenging pigmentation conditions to treat because it has both epidermal and dermal components and is prone to recurrence. Effective management combines targeted in-office treatments with a consistent prescription skincare regimen and rigorous sun protection.

Melasma appears as symmetrical brown or gray-brown patches, most commonly on the cheeks, forehead, upper lip, and chin. It is significantly more common in women, particularly those with darker skin tones, and is strongly associated with estrogen exposure, through pregnancy, oral contraceptives, or hormone therapy, combined with UV stimulation of overactive melanocytes.

At Florida Aesthetics and Wellness, our clinical team evaluates each patient's melasma pattern, skin type, and hormonal history before recommending a treatment protocol. We use low-energy, carefully parameterized laser and light treatments paired with prescription skincare to produce gradual, sustainable improvement without triggering a rebound reaction.

Hormonal Melasma
Often called the "mask of pregnancy," this form is driven by elevated estrogen and progesterone. It may improve after hormonal changes resolve, but frequently requires active treatment to fully clear.
Sun-Induced Melasma
UV exposure is the primary trigger that activates overproductive melanocytes. Without daily SPF 50 and sun avoidance, treated areas can re-pigment quickly, making home care as important as in-office treatment.
Mixed Melasma
Most patients have both epidermal (superficial) and dermal (deep) pigmentation components. The dermal component is more resistant and typically requires a sustained series of treatments rather than a single session.

How we treat melasma

We've organized recommendations by priority, what we most commonly suggest first, and what we layer in for enhanced results.

First Line Most commonly recommended for this concern
BBL HEROic light treatment for melasma at Florida Aesthetics and Wellness Brandon FL

BBL HEROic

Best for: pigmentation reduction, redness, skin tone

BBL HEROic delivers targeted broadband light energy to break up melanin deposits with precision. For melasma, it is used at conservative settings to selectively reduce pigmentation without the heat buildup that can trigger a rebound response. It also targets vascular components that contribute to a ruddy, uneven complexion.

MOXI laser for melasma at Florida Aesthetics and Wellness Brandon FL

MOXI Laser

Best for: gentle resurfacing, deeper pigmentation

MOXI's non-ablative fractionated 1927 nm wavelength is well-suited for melasma because it targets water in the skin to create microcolumns of renewal without the heat associated with ablative lasers. Its low-and-slow approach makes it one of the gentler options for darker skin tones and epidermal melasma.

Common Questions

Melasma treatment FAQ

Melasma results from the overproduction of melanin in genetically predisposed skin, most commonly triggered by hormonal fluctuations (pregnancy, birth control, or hormone therapy) combined with UV exposure. It is considered difficult to treat because the overactive melanocytes sit in both the epidermis and deeper dermis, and because treatments that generate heat or cause inflammation can paradoxically worsen pigmentation. Sustained results require a combination of targeted treatments and rigorous daily sun protection.
Yes, when performed by experienced providers using the right protocols. Traditional high-energy lasers can worsen melasma through post-inflammatory hyperpigmentation. However, gentle fractionated lasers like MOXI, and carefully parameterized BBL HEROic sessions, have shown strong safety records for melasma treatment when combined with a prescription skincare protocol. Your provider will assess your Fitzpatrick skin type and melasma pattern before recommending a treatment plan.
Melasma is a chronic condition. While in-office treatments can significantly reduce its appearance, it can recur, particularly with sun exposure, hormonal changes, or stopping a maintenance skincare regimen. Daily broad-spectrum SPF 50 is non-negotiable for maintaining results. Many patients achieve excellent long-term control through a combination of annual touch-up treatments and consistent home care with Obagi or SkinMedica products.
Obagi Nu-Derm and Obagi-C Rx systems contain prescription-strength hydroquinone (4%) and vitamin C formulations that are substantially more potent than anything available over the counter. Hydroquinone at therapeutic concentrations actively suppresses melanin production in overactive cells, while the full Obagi system promotes cellular turnover to remove existing pigmentation. Over-the-counter products lack the potency and clinical evidence of prescription-grade systems.
Most in-office treatments for melasma, including laser therapies and prescription-strength skincare, are not recommended during pregnancy or breastfeeding. However, a consultation is still valuable, your provider can advise on safe sun protection strategies and help you plan a treatment protocol to begin after you have finished nursing. Some patients find that melasma improves on its own after hormonal changes stabilize.
