THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
1. OUR PLEDGE REGARDING HEALTH INFORMATION
Florida Aesthetics and Wellness understands that health information about you and your health is personal. We are committed to protecting health information about you. We are required by law to:
- Maintain the privacy of your Protected Health Information (PHI).
- Provide you with this notice of our legal duties and privacy practices with respect to your PHI.
- Follow the terms of the notice that is currently in effect.
- Notify you following a breach of your unsecured PHI.
2. HOW WE MAY USE AND DISCLOSE YOUR PHI
We may use and disclose your health information for the following purposes:
- For Treatment: We may use your PHI to provide you with medical treatment or services. For example, we may disclose your information to our practitioners, nurses, or technicians who are involved in taking care of you.
- For Payment: We may use and disclose your PHI so that the treatment and services you receive may be billed to and payment may be collected from you, an insurance company, or a third party.
- For Healthcare Operations: We may use and disclose PHI for our business operations. For example, we may use PHI to review our treatment and services and to evaluate the performance of our staff.
- Appointment Reminders & Service Updates: We may contact you (via phone, email, or SMS) to remind you of an appointment or to provide information about treatment alternatives or other health-related benefits and services.
- As Required by Law: We will disclose PHI when required to do so by international, federal, state, or local law (e.g., public health risks, health oversight activities, or lawsuits).
3. YOUR RIGHTS REGARDING YOUR HEALTH INFORMATION
You have the following rights regarding the PHI we maintain about you:
- Right to Inspect and Copy: You have the right to inspect and obtain a copy of your health records. To do so, you must submit a written request to our Managing Director. We may charge a reasonable, cost-based fee.
- Right to Amend: If you feel that health information we have about you is incorrect or incomplete, you may ask us to amend the information. Your request must be made in writing and provide a reason for the amendment.
- Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures we made of your PHI for purposes other than treatment, payment, or healthcare operations.
- Right to Request Restrictions: You have the right to request a restriction on the PHI we use or disclose for treatment, payment, or healthcare operations. We are not required to agree to your request, except if you pay for a service entirely out-of-pocket and ask us not to share that info with your health insurer.
- Right to Request Confidential Communications: You have the right to request that we communicate with you about medical matters in a certain way or at a certain location (e.g., only at a specific phone number or email address).
- Right to a Paper Copy of This Notice: You may ask us for a copy of this notice at any time.

5. COMPLAINTS
If you believe your privacy rights have been violated, you may file a complaint with:
- Florida Aesthetics and Wellness: Contact our Managing Director at the email or address listed above.
- U.S. Department of Health and Human Services: Contact the Office for Civil Rights (OCR).
- Note: You will not be penalized or retaliated against for filing a complaint.
6. CHANGES TO THIS NOTICE
We reserve the right to change this notice. We reserve the right to make the revised or changed notice effective for health information we already have about you as well as any information we receive in the future. We will post a copy of the current notice on our website and in our facility.
